Requesting Reports:

If you require a lab report, please access your reports in Patient Fusion.

If the report in unavailable or the format is unacceptable, please send a message via Patient Fusion with the specifics about which report you need and why.

Viewing Lab Reports in Patient Fusion:

1. Log into Patient Fusion

2. Click on the "Health Records" tab on the left-hand sidebar.


3. To select another report, click on the report name from the "Lab test results" list on the left-side of the page.

- As you switch between lab results, the content of the right-side of the page will update automatically.

If the report is unavailable or the format is unacceptable please send a message via Patient Fusion with the specifics about what report you need and why.

4. Click on the "Messages" tab on the left-hand sidebar.

5. Click on the "New message" button at the top right-hand corner of the screen.


6. In the new message window, enter the subject of your message into the "Subject" field.

7. Then, enter the specifics about the report(s) you need help with in the body of the message.

- Please include specifics about what report(s) you need and why.

8. Press the "Send" button to send the message to Doctor Novich directly.

Downloading Health Records from Patient Fusion:

1. Log into Patient Fusion

2. Click on the "Health Records" tab on the left-hand sidebar.


3. Click on the "Download >>" link at the top right-hand corner of the screen.

4. Next, choose the download format.

- You will likely want to choose "PDF format".

5. On the "Download PDF Records" pop-up.  Review the options under the "Health records" section you would like to download and check/uncheck boxes accordingly.

6. Additional, check the boxes related to any of the visits in the "Visit Summary" section that you would like to include in the download.

7. Press the "Download PDF Record" button to download your selections to your local computer.

8. Open the downloaded file from your browser or locate the file on your computer and review it.

- The file should be named "patient_summary.pdf" by default.
